This document is to be used for initial referrals, re- evaluations, other evaluations, consent for evaluation and/or services, eligibility, disciplinary review, IEP Record Review, placement and consent, as well as meeting notes. This is to replace the Conference Summary Referral, Initial AND the Conference Summary.

This is a total list of editors available Editors required for locking will vary based on the Evaluation Type selected Evaluation Options Initial Reevaluation Other Evaluation

Initial Evaluation – for the first time a student is being evaluated Reevaluation – for annuals and/or 3 year reevaluations *NEW* Other Evaluation – for any other evaluation that occurs outside of the annual/reevaluations setting, including but not limited to FBA, OT, PT (this will NOT affect the 3 yr reevaluation date)

*NEW* Date for Revocation of Consent of Evaluation & Reason for Revocation of Consent of Evaluation Text box If Revocation Date is entered a Reason must also be entered.

*NEW* Parent/Guardian Information Note the change after the ‘Refresh’ button has been clicked.

Click ‘New Intervention Strategies’ Enter Test Name, scores or info and date the test was given.

After you click ‘Save’ the test will move to the strategies list. All information entered in this editor will print in the format below.

This comes directly from the Summary of Interventions & Data Form with no major changes. Areas allow for unlimited text entries.

Click ‘New Intervention Data’ From the drop list choose a tier of intervention Unlimited text field Limited to 250 characters Begin date is required in order to save this editor

An unlimited number of Interventions may be entered. Print view

Select Area(s) from the drop down list Has a repeatable format allowing all areas to be indicated, however once you choose an area it will not allow you to chose it again but it can be modified

Once you choose an area, you can add as many Reasons for Referring as needed Each list populates based on the area chosen If ‘Other Specify’ is selected additional text will be required.

Notice, after an Area of Concern is Saved, it no longer appears on the drop down list.

The text added to ‘Other, Specify’ will print in the Reason for Referring area as well

Print view Only the areas saved will print to this editor. The only blank box would possibly be Specialized Equipment Used by Student. Any specialized equipment added, will list to the one box, automatically separated by commas.

Up to 5 New Tests can be added.

Passed/Failed drop down box Unlimited text box If yes is selected, text must be entered, 250 character limitation

If the answer is yes, a date and disability must be entered to Save Unlimited Text field

Fields will populate based on which evaluation plan you choose You have to choose one evaluation or the other. If you change after starting, you will loose all of the information entered up to that point.

Once an Area of Assessment is chosen from the drop down list, it will no longer be available to select.

Check boxes only used for SLD Eligibility

Once a report is selected from the drop down list, it is no longer available for choosing. However, more than one service provider can contribute to the same report.

Only the areas you Save or Save & Continue will print. If you save a blank area, you will have a blank area printed under the saved header. You can click through them, just do not save them.

Review of Records

If you select ‘Other, Specify’ you must enter text to clarify

If you choose Multiple Disabilities, you have to list the categories

No matter the assessment, a choice of ‘Other, Specify’ will populate a text box requiring that text be entered. Limited to 250 characters. The ‘Summary’ text box allows for unlimited text entry. Note: In this editor, assessment categories NOT chosen, will appear with a header and ‘N/A’ checked

A first time initial will have a blank box displayed. Select ‘New Checklist’ Select disability(s) that are being determined. You can add as many as needed. Select a ‘Determination’, this is a required field to be saved

The eligibility determination form will populate based on the disability chosen Validations in place will vary by disability determination requirements but may include: only Y or N or Insufficient may be checked If “additional assessments/data needed” is checked, then a date will be required ‘Supporting Evidence’ text fields allow for unlimited text entry

Print View Note: As of July 2015, IC is still working on the Core Product updates to allow cutting & pasting of graphs and objects. This is a Core Product issue not just KY. I will let you know about updates as I am notified.

The only change to this editor is the addition of the SLD “Agree” and “Disagree” check boxes
