The Ratings Numerical scale of 1 to 5 1 = unsatisfactory 2 = needs improvement 3 = satisfactory/meets expectations 4 = exceeds expectations 5 = outstanding
Preparation Review position description Update resume/vita Identify accomplishments - keep notes Establish goals for the next year - employee - department
Facilitate communication Plan for the future - Remove any deficiencies - Improve current skills - Expand personal/professional growth Clarify ratings and expectations Evaluation Discussion
Considerations The employee signature on review form indicates receipt of review – not necessarily agreement All employees may add a written statement/ rebuttal to the annual review Annual reviews are retained in the official personnel file for three years following evaluation date
