 Staff Evaluation Rating template ◦ For Public Schools - each staff person reported must have 4 evaluation records  Local; State; 1 Other; and 1 Overall Composite Score ◦ For Charter Schools – each staff person reported must have an Overall Composite record ◦ Additional field to collect an Alternate Staff ID  In April, a new field was added as the last field in the template  The new field is used to collect the statewide TEACH ID, if your customer is using the TEACH ID as the local staff id  Note: Report either the Staff ID or the Alternate Staff ID, but not both  Evaluation Criteria Code (refer to template for score ranges by category) ◦ STATE - State sub-component score LOCAL - Local achievement sub-component ◦ OTHER01 - Other - Ineffective OTHER02 - Other - Developing OTHER03 - Other - Effective OTHER04 - Other - Highly Effective ◦ OC01 - Overall composite - Ineffective OC02 - Overall composite - Developing OC03 - Overall composite - Effective OC04 - Overall composite - Highly Effective

 New data collection requirements for existing templates: ◦ Student Lite (Demographic)  New Credential Codes for Science and Math ◦ School Entry Exit  Entry Enrollment code for part-time students pursuing a HS diploma ◦ Programs Fact  New Program Service Codes:  SIFE (Students with Interrupted Formal Education)  indication of student’s participation in STEP (Higher Ed)  indication of student’s participation in Liberty Partnership Program (Higher Ed)

◦ Programs Fact (con’t)  New program service ending reason code:  Leave CTE with the intention of continuing later - to identify students who plan to continue the program, but may be skipping a semester or a year.  Remove:  Eliminate Program Type Code for Title II - TPREP. CTE Program Type code will be used for all records. (Participation Info Code) ◦ Assessment Fact  No longer report testing modifications in field: Assessment Modification Code  New Alternate Standard Achieved Code field  New Measure Codes for Career and College Readiness Assessments  New Measure Codes for NYSESLAT ◦ Student Daily Attendance  Used to collect negative daily attendance for : (E)xcused, (U)nExcused, (T)ardy  LEAs will map Local Codes to State Attendance Codes in the dimension table  Multiple daily attendance records for the same date will be reported according to the hierarchy: (1) excused absence, (2) unexcused absence, (3) tardy

◦ Student Class Grade Detail  New Class Detail Outcome Code:  “N”= Not Complete code. Use this code on any record where it is necessary to indicate that the student did not complete the course for any reason (including “drop” or “incomplete”).  New Credit Recovery Code field to indicate credit earned through make-up credit program  “Y”= credit earned through make-up credit program; “N” = credit not earned, or not earned through make-up credit program  Term Code “S” will indicate make-up credit was earned in Summer School ◦ Staff Snapshot  Collection of Teacher information for:  Original Hire Date; (required for all reported staff)  Exit Date; (required when staff leave the LEA)  Tenure Date (required when tenure is granted by the LEA)

 Data collection requirements for new templates: ◦ Staff Assignment  Links principals to groups of students ◦ Contact  Provides Adult contact information and Adult Statewide IDs ◦ Student Contact  Links student to an adult who is authorized to view the educational records of the student
