Local Government Officers are required to file a “Conflicts Disclosure Statement” (form CIS) ONLY IF the following situations occur: (1)The officer or a member of the officer’s family has an employment or business relationship that results in taxable income with a person contracting or seeking to contract with the health district. OR OR (2)The officer or a member of the officer’s family is offered or has received one or more gifts, other than gifts of food, lodging, transportation, or entertainment, with an aggregate value of $250 from a person who is contracting or seeking to contract with the health district.
Local Government Officer Conflicts Disclosure Statement (form CIS) Requirement: File (form CIS) with the district’s records administrator (the Compliance Auditor) not later than 5 p.m. on the seventh business day after the date on which you become aware of the conflict of interest. Penalty: An officer who knowingly fails to file the “Conflicts Disclosure Statement” commits a Class C misdemeanor. Under Section of the Texas Penal Code, an individual adjudged guilty of a Class C misdemeanor shall be punished by a fine not to exceed $500.
