 Immunizations are required to attend school in the United States  The School Nurse, Social Worker, Psychologist, or Guidance Counselor can provide you with a reference list of doctors and dentists in your area  A sports physical examination is required before participation in any sport activity in school

 Medication may be taken at school with proper documentation from a doctor or a parent, as appropriate.  Parents must take the medication and note in to the office. Students may not bring medication in to school themselves.  Only the School Nurse or assigned designee can administer the medication to the student (including cough drops and any over-the- counter medications)  Students must have a pass to see the Nurse (this maintains accountability)

 Read and discuss “From Home to School” Book - “My Head Itches” Lesson 2  Book Level “Literacy” (Green) pp. 8-9; complete p. 10 and the bottom of p. 11  Book Level 1 (Blue) [“An Eye Exam”] pp ; complete pp  Book Level 2 (Pink) pp ; complete p. 12 and p. 14

 Review and explain the differences between medical forms  Role play calling a medical or dental office to make an appointment (PEP Teacher(s) role play first so parents understand what’s expected)  Parents draft a note to send to the school so their child may take a medication

 Go to the Internet and show parents how to find local medical and dental offices  Parents “surf” the Net to find and review health information  Parents type a letter to send to school for their child to take a medicine at school
