Apply the Model for Improvement to your own practice and develop your plan of what to try by next Tuesday...

TEAM PLANNING & SHARING INSTRUCTIONS In this session you will…. a) Use the Aim Statement worksheet to create a written, measurable, and time-sensitive description of the accomplishments your team expects to make from its improvement efforts b)Prioritize multiple tests of change/PDSA cycles over the length of the first Action Period – Use the Improvement Planning Form – Describe each briefly using the timeline c)Develop specific details about initial tests/PDSA cycles you are considering – Use one or the other of the Plan a Test of Change Worksheets – Identify what will you measure to help you identify if your changes have resulted in improvements? Note: A multi-copy form of the Plan Test of Change worksheet will be provided to your team (1 per team), to write the proposed initial test/PDSA cycle Please give a copy of the worksheet to any member of the Project Team. Note: The more well planned your tests are the more likely they’ll be successful
