3 Reminder of opportunity and challenge framework: Opportunities: enhanced federal funding for HCBS services 365 days for qualified individuals certain administrative costs paid % with federal dollars Personnel IT Qualified individuals resided in qualified institutions for 90 days stay is paid by Medicaid at the time of transition move to qualified community residences Challenges: uncertain fiscal environment

5 Framework for discussion of Transition Coordination Definition: standardized set of administrative activities that assist a qualified individual to transition Processes, and roles and responsibilities of staff and agents performing the functions will evolve over the course of the Grant State agencies and their agents will continue to manage Transition Coordination processes that support a broader range of members Today’s focus = qualified MFP members

6 Transition Coordination Tasks Transition Coordination tasks may include: Assessment of service needs non-medical supports medical supports Identification of delivery system options community-based support services community medical providers Securing access to public benefits Development of plans for social activities

7 Transition Processes in MA Given the potential for new resources, how do we: Enhance our current coordination work to: improve success? reach populations not currently assisted? Prioritize or stage enhancements? Enhance IT and data collection? Measure Transition Coordination effectiveness? Handle member complaints about Transition Coordination processes? Determine which processes should be modified, including: prior authorization procedural or regulatory processes

8 Transitional Assistance Components Transitional assistance services described in CF 1115 waiver application, and included in existing HCBS waivers Previous stakeholder input process Can be provided: up to 180 days prior to discharge; and 365 days following discharge Housing Coordination Coordination of efforts to obtain available, affordable and accessible housing for participants

9 Transitional Assistance Components Transitional Assistance Services include such components as: Non-recurring set-up expenses security deposits for lease on an apartment or home essential household furnishings moving expenses window coverings, food preparation items, and bed/bath linens set-up fees or deposits for utility or service access, including: telephone electricity heating water services necessary for health and safety such as: pest eradication one-time cleaning prior to occupancy As well as…

10 Transitional Assistance Components Necessary home accessibility adaptations and/or vehicle modification Adaptive equipment, assistive technology Peer support Activities to assess need, arrange for and procure needed resources: individual support transportation Service animals Family support/training Community re-integration, including: pre-discharge community visits specialized services

11 Transitional Assistance Components Supplemental or Demonstration Services that other MFP States have added, include: 24-hour personal care services housing locator or roommate matching services trial visits to res hab telehealth monitoring or reminders substance abuse treatment (being tested – Texas) cognitive adaptive training (being tested – Texas)
