Go to Gateway landing page Access Access the reporting system by clicking on the Monthly Measures link

Login When you first login, the User Name is your and the Default Password is “quality”. Change Password The first time you sign in you will be prompted to change your password. Please ensure details are correct, and save any changes. Don’t forget to save!

Hint: The past entries screen displays all teams to which you belong and the data that has been entered Data Entry Step 1: Dashboard Check the ‘Action Items’ to see which teams and which months require data reporting.

Data Entry Step 4: Save your data! Note: Enter your monthly data by the 15 th of the next month. Tab to reach the next field. Hint: Hold your mouse over the question marks for measure definitions. Data Entry Step 2: Entering Measures The tabs at the top of your screen represent the teams you belong to. Choose the team tab that you wish to enter data. Data Entry Step 3: Choose the month for which you wish to enter data. Note: You can go back and update the data from previous months at any time. Enter any notes you wish to record, and be sure to press save when you’re done.

Hint: Check the data box to see your team’s numbers for this measure (yellow line), check the Show Aggregate box to compare your scores to the entire action group (blue line) Data Entry Step 6 : Select the measure you wish to see in a run chart. Data Entry Step 7: Save the run chart to.pdf, and print out to review with your team. Data Entry Step 5: Click the ‘Dashboard ‘ tab to go back to view the numbers you just entered in a run chart. Run Charts are automatically updated every time you update your data. Compare your scores to all others in your action group!
