Orientation Forms needed O Health Emergency/Asthma forms (required yearly) O Asthma action plans/permission to carry inhaler (required yearly) O Physical (7 th )required and/or sports clearance (6 th & 8 th ) Important note - a physical must clearly state that student is cleared for sports activities and be signed by the physician to count as sports clearance O Updated immunizations – required 7 th grade tetanus booster O dental form

Important things to know O Orientation occurs sometime during the two weeks before school starts, please check web site for dates and times. O Print and fill out forms from the NMS website and bring to orientation to drop off with nurse O for forms O You can drop off medication and sign permission to administer card

O 7th grade students are required by the state of Nebraska to have their vision, hearing, dental, height and weight screened. This may be done at the doctors office during physical. If the physical is missing this information, vision screening will be done at orientation. Hearing, dental, height/weight screening will be done by the nurse before Dec. 1 unless otherwise directed by the parent. SCREENINGS
