SPC’s Must Complete Sections A & B of the Application

Section A Section A is a one-page executive summary, which includes the following information: 1.List of the organizations to receive PATH funds 2.Service areas 3.Services the federal PATH funds will support 4.Number of clients served

Services Federal PATH Funds Will Support Section 1, page 2 of your Manual for State PATH Contacts lists the eligible services provided through PATH. Additional information regarding the service definitions is available in Appendix B: PATH Service Definitions HINT: Check services listed each year on agency Intended Use Plans to be sure they are listing eligible services.

Section A List of Organizations to Receive PATH Funds Section A can be done in several ways. Some states provide a narrative with all the information included. Others do a table to list the State PATH providers and their allocations You will find examples of Section A in your handouts

Section B Section B consists of state-level and territory information including: The state’s operational definitions of: –An individual who is homeless –Imminent risk of homelessness –Serious mental illness –Co-occurring disorder (serious mental illness and substance use disorder)

Section B (cont.) 2. Estimated number of individuals who are homeless with serious mental illnesses by each region or geographic area for the entire state or territory 3. Description of allocation of PATH funds 4. Description of how PATH funding allocations are consistent with the state’s comprehensive mental health plan

Section B (cont.) 5. Information on whether mental health, substance abuse block grant funds, and/or general revenue specifically serve people who are homeless with mental illnesses 6. Description of provision of programmatic and fiscal oversight 7. Information on whether state provides, pays for, or supports training for PATH funded staff

Section B (cont.) 8. Description of the source of required matching non-federal contributions and assurances of availability of contributions 9. Description of the process for public notice of availability of PATH funding

Section C Intended Use Plans Provide format for PATH programs to follow Provide Budget worksheet for PATH programs to follow Compile IUPs to complete the grant application

Submit to Your Grants Management Group for Review Ask for help Get the grant reviewed for accuracy, spelling and budget correctness Follow through with changes Submit to Governor for signature HINT: Build a timeline so you have plenty of time for the Governor’s office to review. I try to finish my portion 1 month prior to due date.
