ESG Funding for Services to Lakewood Residents who are: Literally Homeless At Imminent Risk of Homelessness
Breathing Room Short-term temporary housing in LCSC’s 2-bedroom apartment Extensive case management services with the goal of increasing housing stability Four milestones to achieve rapid exit from shelter and no return to shelter - Develop an exit plan - Address income needs - Assist with housing search & placement - Resolve utility issues 2016 Anticipate 60 individuals in 18 families will apply Anticipate serving 40 in 12 families
Homelessness Prevention Housing stability and case management Housing search and placement Financial assistance in the form of rent and/or utilities 2016 Anticipate 1,000 inquiries 370 complete application/assessment/receive case management services 260 receive cash assistance
Collaboration with CoC Alignment of Strategies Progressive Engagement for all programs Participate in HUD-mandated Coordinated Intake for the homeless Priority given to homeless families HMIS Data & Reporting Capacity
LCSC Uniquely Qualified 2 licensed social workers with 40 years combined experience with homelessness/mental illness Networks Job Resource Center on site Mental/behavioral health services on site CEOGC outreach worker on site for utility assistance 10+ years of providing housing assistance programs Staff fluent in ServicePoint, the CoC HMIS software
