Performance Plan

Meeting Attendance Child Study: Full Time Staff (>30 hours/week) are expected to attend 85% of Child Study meetings. Part Time Staff (<29 hours/week) are expected to attend 1 Child Study a month. The form in which they will participate will be determined with their supervisor (in person, face time, or by phone) Child Study Facilitators will be taking weekly attendance

Why are we making this change? Child Study is a key meeting in communicating, collaborating and consulting with team members to help the families we serve. We will be shifting the focus of our Child Study to allow for even more collaboration and consulting. “When members attend they receive ideas and strategies from members of other disciplines. If a discipline is missing or leaves the meeting early then the other team members would be denied access to the knowledge and expertise that would have been provided by that person.” Sheldon & Rush Child Study is a time when team members are assigned new families, and given assignments/responsibilities to follow up on with families. Child Study meetings are a key component in serving families and making sure we are in compliance with state/federal requirements. When we miss a meeting families and team members suffer.

Meeting Attendance EI Staff Meeting: Full Time Staff (>30 hours/week) are expected to attend 85% of EI All Staff meeting (1 st Monday of each Month 1:00-3:00 pm) Expected attendance for Part Time Staff (<29hours/week) will be individualized, based on availability, job description, hours worked per week, and determined by supervisors. EI Administrative Assistant takes attendance during the meeting

Why are we making this Change? Training topics are shared during this meeting enabling staff to: Stay current on best practices Develop or maintain skills in training topic Allows for staff to get training hours which is required to renew EI II Credentials Allows for communication on topics that require face to face discussions. Allows for time to recognize and celebrate each others strengths. Allows Time to develop relationships between all EI members who do not see each other on a weekly basis.

Increase the number of Actual Appointments kept from 80% to 82% Services Delivered Per the IFSP

Why are we making this Change? We are anticipating BWEIP to more closely monitor home visit completion percentages. Staff are averaging 83% We have removed the required, 85% as a level 4 expectation. SSIP (State Systemic Improvement Plan) is expecting an increase in positive child outcomes. To accomplish this, we need to ensure we are completing our visits to build parent capacity in meeting the needs of and supporting their child’s development. Taking on new challenges is part of the continuous improvement cycle and it’s how professionals and organizations not only, thrive, but get better and stronger.
