Collection & Packaging Requirements Sample must reach minimum fill line to ensure testing POC cups must rest at least 1 minutes in order to accurately register results Lid must be SECURELY FASTENED. Failure to secure lid may cause leakage and prevent testing Write Patient name and DOB on cup Place collection cup in front pouch of specimen bag with absorbent material Place requisition, face sheet and copy of insurance card in rear pouch of specimen bag and securely fasten bag closed Place secured specimen bags in provided FedEx Clinical Pak Affix FedEx shipping label to Clinical Pak Collection Cup Options 12-Panel Point of Care (POC) Rapid Screen cups1 will be provided to clinic upon request.
