Analysis of admissions: Patients 75+ admitted to all providers 1

Summary of Emergency Admissions The chart on the left shows that the CCG has spent £73m (gross) on emergency admissions for all patients at all providers in the calendar year The chart shows the age splits and the value of the amount spent. i.e. Patients 75+ accounted for the 43% of the total spend on emergency care. The chart on the right shows the CCG’s total number of emergency admissions into hospital by age group in calendar year Patients 75+ made up 30% of total admissions but accounted for 43% of the total amount spent.

Summary of A&E Attendances The chart on the left shows that the CCG has spent £9.3m (gross) on A&E attendances for all patients at all providers in the calendar year The chart shows the age splits and the value of the amount spent. i.e. Patients 75+ accounted for the 18% of the total spend on A&E. The chart on the right shows the CCG’s total number of A&E attendances into hospital by age group in calendar year Patients 75+ made up 16% of total admissions & accounted for 18% of the total amount spent.

High Level Summary Outcomes 4 As per the charts on slide two the CCG spends £30.8m on 11,336 emergency admissions for patients 75 and over across all providers. One of the key outcomes linked to the £5/head investment is to reduce avoidable emergency admissions. The work done by practices/localities as a result of the £5/pt investment will contribute towards the overall target the CCG has to reduce admissions by 15% over the next two years. For practices in Ipswich and East Suffolk CCG this would equate to – 1 admission avoided per week per practice –Over the course of a year this would reduce emergency admissions for the over 75’s by almost 1,900.
