Story Context  As more Americans gain health coverage, hospitals are shifting their charitable dollars towards community health initiatives  The Affordable Care Act’s requirement for nonprofit hospitals to conduct Community Health Needs Assessments (CHNAs) further catalyzes a focus on community health  Most nonprofit hospitals rely on hospital-based data -- such as readmission rates – when conducting CHNAs  Hospitals should look at upstream health factors -- such as access to healthy foods or safe neighborhoods -- to get a complete picture of community health needs 3| © 2011 Kaiser Foundation Health Plan, Inc.November 21, 2015

Kaiser Permanente Solution  Online tool that combines data on environmental and behavioral drivers of health with mapping technology  Over 100 core indicators obtained from counties, public health departments, the Centers for Disease Control and others – Demographics – Income – Insurance status – Violent crime – Physical activity – Soft drink and tobacco consumption 4| © 2011 Kaiser Foundation Health Plan, Inc.November 21, 2015 CHNA.org/kp

Outcomes  Northwest region used mapping tool to get a better picture of community health needs  Previously, charitable funds were largely allocated towards improving chronic disease, access to care and oral health  Mapping tool allowed us to identify maternal and infant health as high-priority issues 6| © 2011 Kaiser Foundation Health Plan, Inc.November 21, 2015
