07/20/2007 Indicator #1 Percent of infants and toddlers with Individualized Family Service Plans (IFSPs) who receive their early intervention services on their IFSPs in a timely manner.

07/20/2007 Definition of Timely Services The provision of all services within 30 days from when a parent/guardian consents to the provision of early intervention services.

07/20/2007 Exceptional Family Circumstances If a timeline cannot be met, it must be documented. The reason for the delay must be based upon the family or a natural disaster. –Examples: hurricane, Intermediate School District (ISD) closed because of a snow storm, child in the hospital, or family emergency.

07/20/2007 Indicator #7 Percent of eligible infants and toddlers with IFSPs for whom an evaluation and assessment and an initial IFSP meeting was conducted within Part C’s 45-day timeline.

07/20/2007 Definition of Initial IFSP Meeting A discussion between the service coordinator, the parent/parents of the child, other family members as requested, an advocate, a person involved in conducting the evaluation and assessments and person(s) who will be providing services to the child and the family. The meeting shall address the possible resources and supports.

07/20/2007 Exceptional Family Circumstances If the 45-day timeline cannot be met because of an exceptional family circumstance, the delay in the timeline must be documented.

07/20/2007 IFSP Completion The IFSP must be completed within 60 days of the referral.

07/20/2007 Indicator #8 Percent of all children exiting Part C who received timely transition planning to support the child’s transition to preschool and other appropriate community services by his/her third birthday including: A.IFSPs with transition steps and services; B.Notification to Local Education Agency (LEA), if child is potentially eligible for Part B; C.Transition conference, if child is potentially eligible for Part B.

07/20/2007 Indicator #8a Transition Steps and Services Review the child’s program options for the period from the child’s third birthday through the remainder of the school year. Discuss procedures to prepare the child for changes in service delivery.

07/20/2007 Indicator #8b Notification to LEA LEA representative signs transition plan or Individualized Education Plan (IEP). Memo or other forms of communication to the child’s appropriate LEA.

07/20/2007 Indicator #8c Timeline for Transition Conference Transition conference may occur up to nine months or at least 90 days before the child is eligible for preschool.

07/20/2007 Indicator #2 Percent of infants and toddlers with IFSPs who primarily receive early intervention services in the home or community settings.

07/20/2007 Justification for Services Not in the Natural Environment When a service will not be provided in the natural environment, there must be a justification to the extent to which the service will not be provided in the natural environment. This should be noted in the child’s file.

07/20/2007 Indicator #5 and #6 Percent of infants and toddlers birth to 1 with IFSPs compared to: A.Other states with similar eligibility definitions; B.National data; C.State target.
