SST Referral Intervention Summary

Data-based Concerns Identify your concerns in regard to an average of your measured/documented information over multiple assessments –Knows 12 upper case and 6 lower case letters –Can read 25/100 Dolch sights words –Student is out of seat 25 times a day –Completes 30% of assigned tasks

Keep a separate record of this data Baseline data must be measurable –Number, percent Student completes 50% of assigned tasks Student knows 13 upper case and 15 lower case letters Student talks out 10 times during a 40 minute class period

Intervention Describe what you have done or are doing to address your concerns –Work 1 on 1 with student –Provide a quiet place in the classroom –Use of an assignment folder –Review letter-sound associations –Shortened length of assignments by 50% –Develop a behavioral contract

This will require separate documentation of the plan –Use flash cards to review and reinforce all letters of the alphabet –Use a study carrel to reduce distractions and provide a quiet place to work – Behavioral Contact – I will … and sign it

Implementation/Duration Precisely describe how each intervention has been implemented –5 days per week for 20 minutes per day –All spelling assignments are shorted to include no more than 8 new words

Must include start and stop dates –Work in small groups (2 – 3 students) 4 times per week, 20 minutes per day 10/12/11 – 12/1/11 –Use study carrel for all independent assigned work 1/2/12 – 2/28/12 –Behavior contract daily 10/27/11 – 1/27/12

Data – based Results Must generate data to document the effectiveness of the intervention –Knows 16 upper case and 12 lower case letters (know 12 upper and 6 lower) –Can read 50/100 Dolch sight words (25/100) –Completes 20%of assigned tasks (30%)

Must keep independent logs of the data so it can be verified The data generated must be a measurable outcome so you can compare to the baseline –Student knows 20 upper and 16 lower case words (13 upper and 15 lower) –Completes 40% of assigned tasks (50%) –Student talks out 7 times during one class period (10 times)
