End-of-Year Review Self Assessment Process May 2014

Learning Targets I can understand the self-reflection process and associated forms. I will know where to access the information I need to complete the self- assessment process and forms.

Form L This is the Form that you will use to self- assess your progress for this year toward your : SLOs Educator Practice Goal Parent Engagement Goal

Completing Form L There is tab with a Textbox and a Rating field for each: SLO Educator Practice Goal Parent Engagement Goal Whole School Indicator YOU DO NOT NEED TO DO THIS! You inherit your building principal’s SLO score for the Whole School Indicator (5%) Just simply ignore that tab on FORM L

For SLOs 1.Review applicable data sets when available. 2.Write in the textbox clear, concise, evidence- based bullets addressing the following guiding statements: a)provide your overall assessment of progress toward the goal/objective and/or action steps, b)describe what you have done that produced these results, c)describe what you have learned and how you will use it going forward, d)what professional learning and/or other type of support would help you to achieve your goals.

Possible Examples... Results on the MAP data indicate... Results on the pre and post unit assessment data indicate... 65% of students met or exceeded the goal. 50% was the target. I attribute these results to... I will continue to __________ because _______ I would benefit from growth opportunities in the following areas... because...

How did we get there? For each SLO: Review the data for each indicator (IAGD) For SLO’s with more than one IAGD, you may score each indicator separately and then average those scores for the SLO score. or you can look at the results as a body of evidence regarding the accomplishment of the objective and score it holistically. SLO 1 IAGDScale Score #1#13 #22 Total SLO 1 Rating = 2.5 Lastly, you take the average of each total SLO rating for your Overall Student Growth and Development score. Example

SLO Rating Scale In general, MET = within 5% of targeted goal.

Calculating Overall Educator Practice  The Overall Rating of MET (3.1) is determined using this scale (see one page handout with scales).

Parent Engagement Goal The Parent Feedback rating should reflect the degree to which you successfully reached your improvement targets and parent goal. This will be accomplished through a review of your evidence and application of the following scale: Exceeded [4]Met [3] Partially Met [2] Did Not Meet [1] Exceeded the goal Met the goal Partially met the goal Did not meet the goal 110% 90% 80% 79% or below
