 Sophomores – Seniors  PDP Reporting Form is due to Dr. O’Neill’s office by Friday SEPTEMBER 13 th  Be sure to get your records together in case you are one of the 20% who get audited.  If Audited ▪ All documentation supporting your PDP reporting form must then be turned in.
