Risk of Acute Events  DOES EXERCISE INCREASE THE RISK OF ACUTE CARDIOVASCULAR EVENTS?  Vigorous physical activity acutely increases the risk of cardiovascular events among young individuals and adults with both unknown and diagnosed heart disease

Risk  Habitual physical activity reduces coronary heart disease events, but vigorous activity can also acutely and transiently increase the risk of sudden cardiac death (SCD) and acute myocardial infarction (AMI) in susceptible persons.

Risk  Exercise-associated acute cardiac events generally occur in individuals with structural cardiac disease.

Causes of SCD  Hereditary or congenital cardiovascular abnormalities are predominantly responsible for cardiac events among young individuals, whereas atherosclerotic disease is primarily responsible for these events in adults.

Sudden Cardiac Death  The absolute rate of exercise-related sudden cardiac death varies with the prevalence of disease.

Incidence  The incidence of both acute myocardial infarction and sudden death is greatest in the habitually least physically active individuals.

Prevention Strategies  No strategies have been adequately studied to evaluate their ability to reduce exercise-related acute cardiovascular events.

Prevention Strategies  Maintaining physical fitness through regular physical activity may help to reduce events because a disproportionate number of events occur in the least physically active subjects performing unaccustomed physical activity.

Prevention Strategies  Other strategies, appear prudent but have not been systematically evaluated: screening patients before participation in exercise, excluding high-risk patients from certain activities, promptly evaluating possible symptoms, training fitness personnel for emergencies, encouraging patients to avoid high-risk activities

Risk-Benefit Analysis  A variety of evidence suggests that habitual physical activity decreases the risk of fatal and nonfatal CAD events and that the benefits of regular physical activity outweigh its risks.

Physical Activity  Consequently, physical activity should be encouraged for most individuals in accordance with the CDC/ACSM recommendations for > 30 minutes of moderate intensity physical activity such as brisk walking on most, preferably all, days of the week.
