Introduction 1st Congress of Southeast European Neurosurgical Society - SEENS2 Midbrain abscesses are rare and were fatal until recently. The optimal treatment is not yet defined. It can be managed by pharmacological treatment alone, stereotactic aspiration with pharmacological treatment, or surgical removal/drainage of the abscess. We applied a neuroendoscopy to treat a large midbrain abscess..Pathohistological examination confirmed cerebral abscess. Conclusion: Purely neuroendoscopic procedure can be a permanent solution for treating large midbrain abscess without complications and with good outcome. Neuronavigation is a useful tool,

Case report A 50-years old male presented with progressive pupil- involving third nerve palsy, contralateral hemiparesis and dysarthria. Magnetic resonance imaging revealed a 2.6 cm x 1.8 cm ring-enhancing lesion in the left side of the midbrain and left cerebral peduncle with associated edema 1st Congress of Southeast European Neurosurgical Society - SEENS3

Case report - operation Neuroendoscopic removal of the lesion was performed through the right lateral and third ventricle using a rigid endoscope with neuronavigation. Pathohistological examination confirmed cerebral abscess. 1st Congress of Southeast European Neurosurgical Society - SEENS4

His neurological condition gradually improved, no new neurological deficits appeared. MRI 2 months after the operation showd no sign of abscess. 1st Congress of Southeast European Neurosurgical Society - SEENS5

Conclusion Purely neuroendoscopic procedure can be a permanent solution for treating large midbrain abscess without complications and with good outcome. Neuronavigation is a useful tool which increases the accuracy and safety of the surgical procedures.
