 Self selected 8 AF4Q Alliances with mature or developing practice coaching programs  Affinity Group Aim Statement: ◦ Identify, highlight and share best practices and tools from the field featuring practice coaching strategies from inside alliance and outside experts.  Monthly webinars focused on practical applications of tools, resources and case studies for practice coaches and practice coach supervisors

 Affinity Group Standardized Practice Coaching Initial Training Curriculum  SC PA Practice Team Maturity Assessment Tool  W NY Return on Investment Calculator  Maine Practice Improvement Service Agreement

 Handout: Practice Coaching Standardized Curriculum provides section overview/content  Source: AHRQ’s “Developing and Running a Primary Care Practice Facilitation Program: A How-to Guide.” AHRQ PCMH website ( ( The guide can be downloaded from the “Highlights” box on the far right side of the webpage ).

 Cohorts of small groups will ‘travel together’ to learn from the tool expert and learn from one another  Our Hypotheses: ◦ Small groups foster better understanding of the tools ◦ ‘Cohorts’ create opportunities to improve networking ◦ Active movement will lessen conference fatigue This is a P-D-S-A !!

 Shot Gun Method (used in golf tournaments to allow play simultaneously)  Participants are assigned to small groups that start at a specified hole and move through the course (tool stations) together  Look at your number given to you upon entry  Find the golf hole associated with number  Group will be given 15 minutes per hole  Groups stay together, advancing to the next tool station
