Time to Change Exercise Consultation on Referral Scheme Walsall Sport and Leisure Development

Time to Change Background Scheme began in 2001 with funding from HAZ, Walsall tPCT, and Walsall Local Authority. In 2003 – The position responsible for co- ordination and delivery was mainstreamed by the local authority In 2007, through Choosing Health Allocation a new position has been funded to assist with co- ordination and delivery.

Time to Change Self Referral and Medical Referral Working with 47 Referral Sources, including Rheumatology, Stroke Rehabilitation, Occupational Health, and Mental Health Meeting in a convenient venue One to one consultation (45mins) Health Screening

Time to Change Barriers to exercise are discussed Motivational Interviewing techniques based on behavioural change theories are used. Activity Plan is produced Unlike other exercise referral schemes, the activity plan does not have to be facility dependant e.g. gyms.

Time to Change The activity plan is individualised – includes lifestyle changes, exercises at home, supervised gym programmes, and specialised sessions. 6 months of support – phone calls and one to one contact

Time to Change Specialised Session; Supervised gym programme- Clients who want to exercise within a gym environment can have supervision and support 100% of the time.

Time to Change Extend – Exercise for the elderly and less mobile. Also stroke rehabilitation Hydrotherapy – Exercise in water, especially for individuals suffering from arthritis. Health Walks – 16 walks (1m – 6m) across Walsall, Daytime and Evenings. Weekdays and Weekends.

Time to Change Walking Programmes – Step-o-meters given to clients on a loan basis Power Breathe Loan – COPD. PACE Sessions (British Association of Cardiac Rehab Phase IV)

Time to Change Heart Care - Two way referral process between Heart Care and the Time to Change Programme. T2CHeart-Care’s G.P Angina Programme and Cardiac Rehab Programme Phase 3.

Time to Change - Successes In 2005 to 2006 – 68% Clients increased exercise levels at 1 month 65% sustained this level at 6 months Diverse range of referral sources Referral Numbers have increased year on year since schemes beginning in 2001.
