Mentoring Communities Basic guidelines per the FOA : Mentor minimum of two communities in Years 3 – 5 –States/Cities/Counties – Preference to communities who have not received CDC funding (Steps, ACHIEVE, PHC, CPPW) –Tribes – mentor 50/50 in and outside tribe At least 10% of funds should be directed to mentoring selected communities

Implementation Guide Timeline By June 15, 2010: Submit proposed PSE strategies to Project Officer By July 15, 2010: CDC confirmation of strategy for your Guide By December 29, 2010: Submit Guide to CDC no (with your Annual Report) Ongoing: Revise/update Guide as needed and submit revised Guide (if applicable) to CDC
