What is Meaningful Use? Meaningful Use is best defined as demonstrating the use of a certified EHR in ways that can be measures as being used in a “meaningful” way. This includes: 1.Using e-Prescribe 2.Using EHR technology for electronic exchange of health information 3.Using EHR technology to submit clinical quality and other measures EPs must comply with certain objectives in three stages. During Stage 1, they must comply with 20 objectives: 15 from a “core set”, plus 5 that they must select from a “menu set” of 10 objectives. © 2013 Insight Software, LLC. All rights reserved.

What is the incentive and what are the penalties? Incentive: 75% of Medicare Part B Penalties: 1%-3% Medicare, %-5% Medicare, There are no penalties for Medicaid providers. Who is Eligible and how do you register? Medicare: Doctors of Optometry Medicaid: Physicians (primarily doctors of medicine) Five states now offer Medicaid EHR incentives to Doctors of Optometry (Alabama, Illinois, Kentucky, Ohio and South Carolina) Medicare & Medicaid Registration and Attestation System You need: An individual NPI number and user account with a National Plan & Provider Enumeration System (NPPES) web user account. 1.Computerized Provider Order Entry (CPOE) Alternate Measure: Based on the total number of medication orders created during the EHR reporting period. An EP, eligible hospital, or CAH may select either measure for this objective. More than 30 percent of medication orders created by the EP or authorized providers of the eligible hospitals or CAHs inpatient or emergency department (POS 21 or 23) during the EHR reporting period are recorded using CPOE. 2.Electronic Prescribing Additional Exclusion: Any EP who does not have a pharmacy within their organization and there are no pharmacies that accept electronic prescriptions within 10 miles of the EPs practice location at the start of his/her HER reporting period. New Stage 1 Changes for Record and Chart Changes in Vital Signs This new measure amends that age limit to recording blood pressure for patients ages 3 and over and height and weight for patients of all ages. New Exclusions: Any EP who: Sees no patient 3 years or older is excluded from recording blood pressure; Believes that all three vital signs of height, weight, and blood pressure have no relevance to their scope of practice is excluded from recording them; Believes that height and weight are relevant to their scope of practice, but blood pressure is not, is excluded from recording blood pressure; or Believes that blood pressure is relevant to their scope of practice, but height and weight are not, is excluded from recording height and weight. © 2013 Insight Software, LLC. New Stage 1 Changes for Electronic Exchange of Key Clinical Information Beginning in 2013, the objective for electronic exchange of key clinical information will no longer be required for Stage 1 for EPs, eligible hospitals, and CAHs. 5.Report Clinical Quality Measures CQM is now a separate objective for reporting ambulatory or hospital clinical quality measures as a part of meaningful use. EPs, eligible hospitals, and CAHs will still be required to report on clinical quality measures in order to achieve meaningful use. CMS is removing the standalone objective that requires providers to attest that they plan to report on CQMs because it is redundant. © 2013 Insight Software, LLC. All rights reserved.

CPOE for Medication Orders  Objective: Use computerized provider order entry (CPOE) for medication orders directly entered by any licensed healthcare professional who can enter orders into the medical record per state, local and professional guidelines. Measure: More than 30 percent of all unique patients with at least one medication in their medication list seen by the EP have at least one medication order entered using CPOE. Maintain Problem List  Objective: Maintain an up-to-date problem list of current and active diagnoses. Measure: More than 80 percent of all unique patients seen by the EP have at least one entry or an indication that no problems are known for the patient recorded as structured data. Measure: More than 40 percent of all permissible prescriptions written by the EP are transmitted electronically using certified EHR technology. Record Vital Signs  Objective: Record and chart changes in the following vital signs: height, weight and blood pressure. Calculate and display body mass index (BMI). Plot and display growth charts for children 2-20 years, including BMI. Measure: For more than 50 percent of all unique patients age 2 and over seen by the EP, height, weight, and blood pressure are recorded as structured data. Exclusion: Any EP who either sees no patients 2 years or older, or who believes that all three vital signs of height, weight, and blood pressure of their patients have no relevance to their scope of practice. Physician Quality Reporting System? Beginning in 2012 and continuing in 2013, there are two reporting methods available for reporting the Stage 1 measures: 1. Attestation 2. e-Reporting Pilots: Physician Quality Reporting System EHR Incentive Program Pilot for EPs eReporting Pilot for eligible hospitals and CAHs Physicians who qualify, may earn a bonus payment on all allowable Medicare charges = 0.5% 2014 = 0.5% A penalty of 1.5% for not doing PQRS begins in A penalty in 2015 will apply depending on 2013 performance. © 2013 Insight Software, LLC. Electronic Copy of Health Information  Objective: Provide patients with an electronic copy of their health information (including diagnostic test results, problem list, medication lists, medication allergies) upon request. Measure: More than 50 percent of all patients who request an electronic copy of their health information are provided it within 3 business days. Exclusion: Any EP that has no requests from patients or their agents for an electronic copy of patient health information during the EHR reporting period. Clinical Summaries  Objective: Provide clinical summaries for patients for each office visit. Measure: Clinical summaries provided to patients for more than 50 percent of all office visits within 3 business days. Exclusion: Any EP who has no office visits during the EHR reporting period.
