Questions Our health department does not have a close relationship with our hospital. Who at the hospital should I talk to about working together on a community health assessment?
Questions How can we engage hospitals that are not interested in completing a comprehensive community health assessment?
Questions Any advice on how an LHD and hospital can co-lead an assessment process? How can we ensure that one entity is not disproportionately burdened with all the work?
Questions How can we engage competing non-profit hospitals in our assessment process?
Questions Given that non-profit hospitals will have to complete an assessment every 3 years per ACA and health departments will have to complete an assessment every 5 years per PHAB, how can we align efforts so hospitals and LHDs are well-positioned to meet their respective requirements?
Questions How can you get hospitals to release de-identified data for a joint data collection process in which public health and the hospitals work together for a joint community health needs assessment? It seems the hospitals are very protective of their data and sharing with other agencies has been an issue.
Questions What type of data sets do you use for a joint data collection process? Do you have a template to share? work/clearinghouse/phase3CHSA.cfm See CHSA Core Indicator Set Worksheet and CHSA Extended Indicator Set Worksheet
