National Chapter Award Recognition for Your Success!

Purpose Encourage the development and use of a Program of Activities (POA) Reward chapter accomplishments Recognition for chapter success

Awards Superior Chapter Certificate –Complete Form I, State level Gold, Silver, Bronze multi-year plaque –Complete Forms I & II, State level Three, Two, One Star multi-year plaque –Chosen from Gold level applications sent on for national competition

Awards Models of Innovation –National Three Star rated chapters eligible to compete for this award –Ten finalists in each of the three divisions Student, Chapter, Community –One winner in each division is selected at convention through a presentation/interview process.

Form I Lists and briefly describes one activity for each of the fifteen quality standards. –Student Development leadership, healthy lifestyles, SAEs, scholarship, agricultural career skills –Chapter Development chapter recruitment, financial, public relations, leadership, support group

Form I (cont.) –Community Development economic, environmental, human resources, citizenship, agricultural awareness Transfer information directly from your completed Program of Activities.

Form II Complete for State Recognition-Gold, Silver, Bronze 3 or a number equal to 10% of all chapters may be sent on for national competition. All sent on must be rated Gold. National Awards of 3 Star,2 Star, or 1 Star

Goals/Objectives What did you want to accomplish? Overall objective of the activity –Purpose or reason for involvement Goals of activity –Three or more specific, measureable, realistic and attainable goals.

Which standard does it fit? When deciding which standard an activity fits best, consider the goals of the activity. What group was being encouraged to develop skills, who was benefiting?

Plan of Action Who, What, When, Where, Why & How Is the plan educational and will it meet the goals of the activity? What ideas were used in the planning process? Is the plan complete? (address each goal)

Results/Evaluation What was accomplished? –Were the goals achieved? (address each goal) –How did this activity benefit or impact the student, chapter, school or community? –What was learned? –Who participated?

Photo & Caption A high quality picture, showing involvement in the activity. 50 word caption that is informative and interesting Do not repeat photos within the application.

General Tips Be sure to get all signatures Use only the space provided and do not attach additional materials to the application. Form I should have 15 different activities, Form II should have 9 different activities, which may come from Form I.

General Tips Use size 12 pica Protect application in some type of binder Print application on white paper Use upper and lower case letters
