Ventura County Continuum of Care Coordinated Entry System and Housing First

Phase II CES Implementation Training on VI-SPDAT VI-SPDAT completed with chronically homeless individuals 35 surveys completed and entered into HMIS Development of Policies and Procedures Matching most vulnerable individuals with Permanent Supportive Housing

Matching for Housing 3 SRO units with Community Action (5 more to be identified) Top 3 individuals matched!! All matched individuals have agreed to housing Clients with severe health needs, homeless for years, frequent hospitalizations, easily taken advantage of by family/friends Plan to offer/provide appropriate supportive services
