3 rd iFAD 30/11/2013Indirect Calorimetry 1 P11: Comparison of indirect calorimetry with Quark RMR and M-COVX calorimetres in critically ill patients 30th November rd iFAD– Poster Session 3 Valérie Vanbiervliet Van Biervliet V, Van Regenmortel N, De laet I, Schoonheydt K, Dits H, Malbrain MLNG ICU, ZNA Stuivenberg, Antwerp, Belgium

3 rd iFAD 30/11/2013Indirect Calorimetry 2 Indirect Calorimetry Measurement of Resting Energy Expenditure (REE) Non-invasive bedside method Breath-to-breath analysis through gassampling Spontaneous and mechanically-ventilated patients Gold standard = Deltatrac Metabolic Monitor New methods? Quarck (Cosmed, Rome, Italy) M-COVX (Datex Ohmeda, Helsinki, Finland)

3 rd iFAD 30/11/2013Indirect Calorimetry 3 Study Protocol Patients and Methods:  Retrospective analysis of data on 11 measurements obtained in 8 mechanically ventilated patients.  Exclusion criteria:  FiO2 > 0,6  Thoraxdrain, or ventilatory leakage  Renal replacement therapy Aims: Comparison of accuracy in the calculation of daily caloric needs and REE with two devices, the M-COVX and the Quark RMR.

3 rd iFAD 30/11/2013Indirect Calorimetry 4 Results APACHE20,1±7,7 SOFA6,6±3,1 Body Weight78,6±12,2 kg Height171,4±7,5 cm Body Mass Index26,8±4,2 Body Surface Area1,9±0,2 m² Ideal Body Weight64,9±8,6 kg Lean Body Weight55,6±7,4 kg

3 rd iFAD 30/11/2013Indirect Calorimetry 5 Results cont. Pearson Correlation P<0,01 – M-COVX = 0,46 x Quark kCal – Coëfficient of Variation = 14,2%

3 rd iFAD 30/11/2013Indirect Calorimetry 6 Results cont. Bland and Altman Analysis – Mean Bias = 243±239 kCal – Lower Limit of Agreement = -236 kCal – Upper Limit of Agreement = 722 kCal – 28,3% Percentage Error

3 rd iFAD 30/11/2013Indirect Calorimetry 7 CONCLUSIONS Indirect calorimetry with M-COVX has a significant correlation with the Quark RMR, however with the measurement with the M-COVX shows a systematic overestimation of 243 kCal of the REE daily caloric needs in comparison with the Quark RMR.
