Applicability Eligible Hospitals Critical Access Hospitals (CAH) Electronic Copy of Discharge Instructions

Training Objectives Configure RPMS & EHR to provide patients with an electronic copy of their discharge instructions upon request. Review newly created TIU note titles that document requests for electronic copy of discharge instructions and fulfillment of the requests. Electronic Copy of Discharge Instructions

MU Measure Type of Measure: Rate The number of patients in the denominator who are provided an electronic copy of discharge instructions. >50% Number of patients discharged from an eligible hospital's or CAH's inpatient or emergency departments (POS 21 or 23) who request an electronic copy of their discharge instructions and procedures during the EHR reporting period. Electronic Copy of Discharge Instructions

Threshold More than 50% of all patients who are discharged from an eligible hospital or CAH inpatient or emergency department (POS 21 or 23) and who request an electronic copy of their discharge instructions during the EHR reporting period are provided it. Electronic Copy of Discharge Instructions

RPMS Reporting Logic Denominator Inclusions: COUNT: each patient HAVING: requested an electronic copy of discharge instructions during the EHR reporting period HAVING: a hospitalization during the EHR reporting period (defined as Service Category of H) WHERE: the Discharge Type was Regular Discharge, Transferred, or Irregular Discharge OR HAVING: an emergency department visit during the EHR reporting period (defined as clinic code of Emergency Department-30 and Service Category of A) Electronic Copy of Discharge Instructions

RPMS Reporting Logic Numerator Inclusions: COUNT: each patient in the Denominator WHERE: the patient was given an electronic copy of discharge instructions at the time of discharge Electronic Copy of Discharge Instructions

RPMS Reporting Logic Measure Exclusion: An eligible hospital or CAH that receives no requests for electronic discharge instructions during the reporting periods is excluded from this requirement. Electronic Copy of Discharge Instructions

Configure RPMS During our how-to session we will: 1.Create or edit inpatient discharge instructions. 2.Document that the electronic instructions were or were not given. Electronic Copy of Discharge Instructions
