Aims of the Project Increased awareness of transition process to colleagues and stakeholders No gaps in between services Ensuring young people have ownership of their transition

Successes Stakeholders Engagement GP’s Dr Ayegba, Clinical Lead CCG On board and supportive Adult Mental Health Services Dr Gary Fahy, Adult Consultant Psychiatrist is now involved and part of the project team. Learning Difficulties LD colleagues remain on board.

Young Peoples Focus Group Young Person Ambassador remains an active member of the project group, attending 2 weekly meetings. Link with Council of Governors. Link with SHOUT (young persons participation group) in Knowsley.

Challenges Increase of number of referrals and capacity and demand issues in the team with an increase on waiting list. Management restructure - new Assistant Director for Knowsley will be in post April Timelines working towards

Next Steps “So what measures” Reduction of Harm Safety -Use of ‘CHI’ questionnaire - DNA Rates to decrease to look at patient experience- Increase in awareness in other services about transitions -Less complaints/more compliments -Have people stayed in service after transition -Opportunities in new structure -Specific transition join clinics – virtual mini team -Development of monthly meetings – this will create established relationships, common agendas and clinical case discussions. -Half day engagement event in the Knowsley borough to discuss new transition pathway and agree vision for transitions in the borough.
