Introduction Health disparities continue to grow within the community and the city of Detroit, Michigan would benefit from a wellness center targeting vulnerable populations such as the homeless and uninsured. An environmental scan provides strong evidence to support the community wanting and needing access to health promotion services. Figure 2. Homeless(Michigan Radio, 2014)

Mission Statement To provide homeless citizens and other vulnerable populations the opportunity to receive safe and professional health services while helping to promote self-responsibility and enable individuals to gain control over their own health and wellbeing. Research local shelters that would most benefit from mobile care service. 2. Develop staffing and equipment budget. 3. Obtain a vehicle that is able to transport staff and equipment to established locations. 4. Travel to residential homeless shelters in the community and provide free health screenings to at risk populations. 1. August March August September 2015

Establish consistent dates that mobile care will be available in the selected areas. 2. Provide clinic hours that are convenient for community members to return for health care services. 3. Establish reminder services through homeless shelters in the community for follow-up visits. 4. Place flyers throughout the community advertising the center hours and future mobile care dates and times. 1. August May June July 2015

Place advertisements in local newspapers and develop flyers to disperse through the community for volunteer services. 2. Contact local nursing and medical schools for potential students requiring completion of community service hours. 3. Interview applicable candidates for volunteer positions. 4. Hire five volunteer candidates to assist with wellness services. 1. March March April May 2015

By joining Discovering Life, a nurse is not only giving back to the community and developing a standard of care that will make a life- long impact on those in need, but also fulfilling the idea and vision of what nursing was founded upon. Requirements: Associate or Bachelors degree in Nursing with licensure to practice in the State of Michigan. Minimum 2 years of clinical experience. Specialties preferred. Conducts patient assessments, administers medications, documents in a timely and accurate manner, and provides follow-up care. Measures health outcomes against patient care goals and standards; makes or recommends necessary adjustments. Follows the community center and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. Promotes patient independence by establishing patient care goals. Teaches patient and family to understand their condition, medications, self-care skills, and answers all follow-up questions.

Budget Proposal

Conclusion Vulnerable populations, such as the homeless, experience significant disparities in access to and the use of healthcare services, life expectancy, morbidity, and mortality (Martins, 2013). Investing in the vision of Discover Life Community Wellness Center will create a new hope in the city of Detroit for health services. The proposed plan for the community center is attainable and sustainable with the help of investors funding. The proposed plan figures in needed health services, revenue resources, and a budget realistic to garnered support. A community that comes together, thrives together.
