Modernising Primary Care for 2020 Ambition “by marshalling all available resources around our citizens we will support their health and well being, coordinated from Primary Care” Philosophy “working together with kindness, respect and honesty in every interaction” From within primary care we will build continuous relationships with citizens and partners to allow a greater level of local coordination of resources. We will make the patient experience as personal, supportive and appropriate as we would wish for ourselves. We will create the conditions for everyone to take their share of responsibility for health & well being. We will create a community team that reflects our ambitions; to extend the existing team to include other community, hospital, partner resources. We will create a richness of trust and understanding between citizens and the NHS through regular, meaningful conversation. Statement of Intent
