Improving Lives – Why? Patients want to be in control Lack of integration across system Poor outcomes, high cost Limited provision for non complex patients Hospital admissions/readmissions Intermediate care is predominantly bed based Ageing population 2

Commissioning Quality Care Patients at the centre Commissioned Experience Led Commissioning Engaging with a range of patient and carers about:  Their current experience  What matters to them most  What a good experience looks like Will directly influence the patient experience outcomes included in the contract. 3

Commissioning Quality Care Clinicians Governing Body reviewed all of the evidence Vision for future aligned to patient views Consulted with and supported by the 19 membership practices Tested with providers through market testing exercise (PINS) 4

Commissioning Quality Care Prime Contractor Model Contracting vehicle to drive improvements Transformation and integration of supply chain Payment for achievement of outcomes 7 year NHS contract to enable them to effect change 5

Commissioning Quality Care So what will be different? Improved outcomes for patients and their carers More people supported earlier Patients and carers will be active partners in their care Modern delivery methods, increased choice & accessibility Integration of service provision across health, social care and the voluntary sector 6

Commissioning Quality Care Timescales Improving Lives Procurement now live Intend to award a contract to a preferred bidder by March 2015 No change to existing contracts/services at this point in time CCG will work with prime contractor to undertake consultation at point of proposed change. 7
