School Questions to Explore Percentage of students who are eligible for free or reduced lunch program Volume of students who could benefit from the program What is the school district’s process to approve-(i.e. School board, Building principal, Superintendant) Dedicated Confidential Room with locked file cabinet Occupancy Permit

Questions to Ask Provider How many staff will provide services? Will different staff be assigned to different buildings? What time frame will staff be available? Will family treatment be available after school hours and during the summer? Will staff be credentialed with commercial insurance plans?
