A quarter of the population in Scotland is taking four or more drugs and most of these patients are over 50 years old. In Glasgow this figure is higher. Adverse reactions to drugs are a factor in 5- 17% of hospital admissions of older people

Transformation Programme Building on the work established under the Change Fund programme This year 32 diverse local and city-wide Third Sector organisations will focus on: Improving the health of older people and/or their families and carers Reducing social isolation of older people Increasing capacity and resilience of older people and/or their families and carers Building the capacity of individuals and communities to be more connected Some funded services will work mainly or partly with older people and carers with whom they had already engaged through their previously funded transformation activities. Power of attorney is for when you are still here. If you are in hospital or a care environment or even your own home, no one can act for you – pay bills, manage your welfare or make key decisions unless you have made your plan. Think and act now: If ill health or an accident means you can’t make decisions for yourself, who do you want to make them for you?
