NHS Health Check National cardiovascular risk assessment programme which aims to prevent heart disease, stroke, diabetes and kidney disease and raise awareness of dementia. In April 2013 became mandated public health service in England. Local authorities have legal duties to Offer health checks to eligible people once every 5 years Deliver health checks Ensure individuals are told their results & CVD risk score Information is forwarded to GP (when HC conducted outside GP) Improve uptake rate Report local NHS Health Check data to central government NHS Health Check Programme

NHS Health Check Main preventable risks leading to death for the East of England The main risks leading to death are high blood pressure, smoking, high cholesterol and obesity. NHS Health Check Eligible population: 275,335 (over 5 years 2013 – 2018) Annual target offers: 55,000 Actual offers: 42,049 Annual target delivery 36,300* Actual delivery: 22,293 * Based on 66% uptake Norfolk Performance

NHS Health Check Pharmacy delivered 2,295 10% of NHS Health Checks. Pharmacy provision varies over the year 6 – 8% in Q1 – Q3. In Q4 17% checks delivered by Pharmacy Norfolk Pharmacy NHS Health Check delivery

NHS Health Check Most patients use local pharmacy (with exceptions in central Norwich and KL) Activity level – huge variation between pharmacies Some pharmacies doing health checks on younger men (key group). Some pharmacies are referring to Lifestyle services Pharm Outcomes observations
