SignatureCare 500 Base Policy More Benefit Provisions… Coverage Outside the U.S.: This benefit is payable subject to certain limitations and up to the daily and lifetime maximums for the benefit outlined in the policy Additional per occurrence elimination period Waiver of Premium: Facility services only and comprehensive policy Waiver continues if facility services or HCBS (if covered under the policy) are being paid. Any unearned premium will be returned on a pro-rata basis.

Not For Use With Clients. 15 SignatureCare 500 Base Policy Daily Benefit Amount – the maximum amount to be reimbursed to the insured for facility services or HCBS on a daily basis. $50 - $400 ($500 in AK, CT & NY) per day. Increased in increments of $10. $50 $400 Daily Benefit Amount

Not For Use With Clients. 17 SignatureCare 500 Base Policy For each true statement write a “T” in the box and for each false statement write an “F” Let’s review… SignatureCare 500 offers both facility only coverage and comprehensive coverage. SignatureCare 500 offers a zero days elimination period as part of the base policy. SignatureCare 500 has a two year benefit period option. The maximum daily benefit amount on the SignatureCare 500 policy is $300. ‘T’ or ‘F’

(25% in NY) Partner Discount - 1 5% applied to policy in all years when only one partner applies, or both partners apply and one is declined. Loyal Customer Discount – if you are currently insured by or own a life insurance policy, disability insurance policy or certain annuity contracts through MassMutual, or have an application pending, you may receive a 5% discount each year on your LTC insurance premiums. Multi-life (Employer/Association) Discount - 10% applied in all years when the policy is purchased through an employer or association. *Discounts are subject to state approval and may not be available In all states
