Health and Wellness Use of Company Equipment Cumberland Heights

Cumberland Heights offers voluntary recreational, social, community and education opportunities to support employee health and wellness.  The following slides provide an overview of the general guidelines and safety precautions to ensure a safe environment and for employees to have fun while having fun. The policy and waiver form can be found on the Policy and Procedures page of the intranet: under the Human Resources chapter.

Waiver of Liability/Informed Consent lets get the legal stuff out of the way first…  Each employee shall complete and sign the wavier of liability /informed consent form before utilizing or participating in any company-provided facilities, equipment and other property related to these facilities. It is a general liability form that acknowledges voluntary participation in physical activities and use of company-provided facilities.

General Guidelines You gotta behave…  Don’t be disorderly or inappropriately use the company facilities, equipment and other property.  Activities should not disrupt normal business operations.  No vandalizing or defacing of facilities, equipment or other property.  Don’t remove equipment or other company materials without prior permission.

General Guidelines Still behaving…  Wear appropriate and acceptable clothing and shoes ~ teaser ~ more on this later…  Be kind and help clean up  Report any defective, damaged and/or other maintenance issues to the Recreation Therapist and/or Director of Support Services  Don’t bring your bike, skates, scooters, skateboards, or other sports equipment without asking for permission first.

General Guidelines Really! There’s more…..  Smoke or use your tobacco before or after and only in designated areas. Not at the event.  No possession or consumption of alcohol or illegal drugs or being under the influence of these in or around Cumberland Heights’ facilities, equipment or other property.  Kiddos under the age of 18 should not be in the fitness/workout room without the direct presence and supervision of a responsible adult over the age of 18.  And, any individual over the age of 18 who is not able to care for his/herself also needs direct supervision to be in the fitness/workout room.

Caution This is voluntary…  The following activities pose potential safety risks for injuries. Take care to prevent injuries.  Basketball/Volleyball court and equipment  Climbing wall and equipment  Fitness/workout room and equipment  Baseball field and equipment  Frisbee Disc course and equipment  Walking trail and fit station equipment Guidelines will be posted in these areas and any injury should be reported immediately to a supervisor and Human Resources. Potential injuries could include: sprains, bumps, bruises, scrapes, scratches, cuts, and/or broken bones Other effects could include: exhaustion or dehydration Take time to stretch before and after attempting any activity. Drink plenty of water before, during, and after any activity. No “horse-play” & don’t participate in any activity that is beyond your functional ability.

Dress Code: Dress for success…  Pants and/or shorts must be worn at the waistline and not below  T-shirts and/or tank tops must be worn at all times  Bare midriffs are not acceptable  Gotta wear shoes – no bare feet; can’t do open-toe shoes, sandals, dress shoes, flip flops or other non-athletic shoes either.  Boots or black-soled shoes are not permitted on the basketball court or in the fitness/workout room.

Violations Oh, you’ve done it now…  Direct violations of any of the above guidelines or failure to comply with these guidelines will result in either a warning and/or loss of privileges for the area.  Leadership hopes you enjoy and will partake in the health and wellness activities offered throughout the year. Please complete and sign the waiver/release form and return to Staff Development. Now your part You’ve got some work to do…
