This document must be filled out and on site prior to entry into a confined space.

This program supplements the company CSE program and must be used whenever entry is required

This person is responsible for entering the space and performing the required work

This person is positioned at the entry point but is not allowed entry. Maintains constant contact with the entrant. Monitors conditions, and has overall control of tasks.

These 3 things must be present in order for a space to be considered a “confined space”…. (must provide all 3 correctly for credit)
