Multi-agency, multi-disciplinary locality Family Support teams

Referral and Allocation Process Via e-CAF By phone Self-referral – ask family to go to nearest HUB Centre and ask to speak to a Family Support Worker  Case allocated (team nearest to where family live)  FSWs assess family and child’s needs (home visit)  Letter to network confirming allocation of case and name of FSW  At close of case - Outcomes Summary sent to network  Re-refer at any time
