5 Past Due Notice Payment Past Due 01/25/2009 Dear Contract Holder, This is to inform you that your Surcharge Payment is Past Due to the SEWP Office. Your last payment was check# XXXX, dated 01/01/2009. Per the SEWP Contract, you are responsible for sending in your payments on a quarterly basis. Please let me know if you think you have received this message in error or if there are any issues concerning your payment, before it affects your Past Performance status. 6 Past Performance Scale  First Late Notice – from the SEWP Finance office  Second Late Notice – from the SEWP Finance office and Al Marshall will be calling you about a solution before your Past Performance is move down in color  Third Late Notice – Past Performance will move down one color NOTE: Every month past the Third Notice will reflect another move down in Past Performance until the Surcharge has been paid.

7 Past Performance Scale Continued Each Quarter that your Company pays the Surcharge payment on time, they will move up a color.
