Community health services review process Exploring advances in healthcare Focus on developing our ten community hospitals Moving care more into the community where this is practical and clinically safe Local groups in each locality during proposals What new services needed (population needs assessment) How existing services could be provided in a different way Assessment of capital and service requirements Two way discussions with local people and groups throughout the review

Public consultation 16 June – 5 October 16 weeks because of holiday period Overall vision and proposals for each area Ashby and Lutterworth proposals go to public consultation at a later date to allow time to test new models of care Report from November 2008

Overall vision ‘Hub and spoke’ model – extended range of services in five ‘one-stop hubs’ and ‘spokes’ offering range of services in other community settings More support for patients to stay in their homes as long as possible Two Walk-In Centres – existing one in Loughborough and additional 8am-8pm centre in south Leicestershire Care for minor injuries and illnesses through a range of options in each locality led by primary care More diagnostic tests, outpatient clinics and day case surgery locally Extended palliative care and rehabilitation services

Hinckley ‘ One-stop hub’ Maintain inpatient beds Expand theatres, outpatient and diagnostic services Move services from Hinckley and District Hospital to one site at Hinckley and Bosworth Community Hospital Relocate GP Health Centre from Hinckley Health Centre to the Community Hospital Sell Hinckley and District Hospital
