History: Phil T. Rich is a 26 year old male with Autism and is non-verbal. Phil takes the county bus to the adult day workshop, where he attends full time. Scenario: Phil gets on the bus this morning and begins hitting his head. Once he arrives at the workshop, Phil begins working on his daily task of packaging nuts & bolts. He is much calmer and attentive. The individual next to Phil begins yelling very loudly. Phil begins hitting his head and throwing parts at the person next to him. Staff attends to the situation quickly by yelling at Phil to stop. Phil begins to pace back & forth hitting his head. He was escorted to the quite area to calm down. Later Phil went to lunch where the cafeteria was very busy. He chooses to eat lunch with one other person in a separate room. No incidents occur at this time. After lunch, staff take Phil to listen to music with a large group in a small room. Phil begins hitting anyone close to him. Staff escort him out of the room and perform a basket hold. Later staff get Phil ready to go home and walk him to the bus. Phil begins to hit his head once he is seated on the bus with the rest of the passengers.
