Royal Derby Hospital: Operational Efficiency Through Partnership Chris Blunsdon Transformational Team, Derby Teaching Hospitals NHS Foundation Trust

Who am I?  Worked in the health service for 43 years  Started as a Cadet Nurse in Carlisle when I was 16  Qualified as a Nurse in Carlisle  Moved to Derby and trained as an ODP  For my sins became Theatre Manager  I retired from my post as Theatre Manager in April 2015 and started working in Planned Care Transformation on some specific projects pertaining to Theatres.

Stress / Pressure We’ve all been here !

Strapped for cash

How can you progress and improve when there is ‘no money’? 1.Business case - Wait for NHS to make money available. Slow process 2.Charitable funds - National / Local charities, Long process, NHS Lottery? 3.Partnership - Foster Industry relationships What are the routes to acquisition of equipment?

Drivers to acquire OR1 Theatres  State of the art equipment required to remain competitive in the region  Capitalise on the expertise of the existing surgeons  Forward-looking aim of attracting additional top surgeons

Drivers to improve training  Live operating for educational courses  Close links with clinical skills department  Live links to lecture theatres in education centre required for medical students  Laparoscopic workshop equipment provided

Process for Managed Service  OR1s purchased July 2010  Need for a managed service was established  Managed Service commenced July 2011  On Site Technician employed as part of contract

A step into the unknown  Apprehension of how the role would be implemented  What exactly would be required of theatre staff and On Site Technician?  What are the responsibilities for existing equipment?  How will the On Site Technician integrate into the team?  Training – would staff lose skills?  Theatre Staff −“What are you selling?” −“What will you do all day?” −Extra pair of hands in theatre? (No patient contact)

On Site Technician - A UK first Objectives  Relationships  Flexibility  Impartiality  Ensure good performance and manage risk  Visits from other Trusts

On Site Technician - Role  Trust induction −DBS check −Occupational health clearance −Honorary contract  Daily duties −Preventative maintenance checks and set up −Troubleshooting −Training −Equipment utilisation −Data management −Asset management −Course support −Educational videos −Liaison

Relax, it’s taken care of  The ‘STORZ man’ will sort it!  Allowed us to concentrate more on what we should be doing – this was invaluable  Increased confidence of staff due to training  Enabled better planning - more patients could be treated due to the reliability of the equipment  Quarterly meetings update

Need for Expansion  Urology rigid endoscopy sets old and in poor state of repair  No capital funds  Quotes prepared with the aim to rationalise portfolio  Instead of replacing everything, fewer sets were required due to ongoing care of kit and higher performance  12+ months of negotiation internally and with KARL STORZ  Kit was commissioned January 2014  Theatre training and support in theatre

Further Expansion  New stack systems and GI scopes added to contract  Existing camera systems covered for maintenance  ENT/Max Fax O/P Camera systems and nasoendoscopes  Second OST required due to volume of covered equipment  Theatre manager involved in recruitment of new OSTs Becky and Aidan

A Happy Team!

Thank you
