Institute of Medicine Report Estimated 98,000 people die every year at a cost of $29 Billion dollars from preventable medical errors. If the Centers Disease Control were to include preventable medical errors as a category, it would be the 6 th leading cause of death in America. 2

Five Diamond Approach to Patient Safety Program Assists dialysis facilities to improve both staff and patient awareness of specific patient safety areas, promotes patient safety values, and builds a culture of patient safety in every dialysis facility. 3

Patient Safety Culture Definition : a set of values, beliefs and behaviors that a group of people at each level within an organization share with respect to safety Benefits:  Employees are less likely to engage in unsafe acts  Advances in safety performance –includes increased staff participation and safety behavior- like compliance  Promotes accident prevention

Patient Safety Culture Continue How do we achieve a Patient Safety Culture?  Based on employee’s perception of the policies, procedures and practices relating to patient safety.  High level of management and physician involvement  Focus on Safety Education with the facility  Open and reciprocal communication between staff and management  Communication style is based on support and is blame free

Five Diamond Approach to Patient Safety Objectives :  To promote patient safety values  To create an awareness of patient safety issues  To help dialysis units learn more about specific areas of patient safety  To build a patient safety culture in every dialysis unit

Details  The application to register is on the website  Patient Safety Principles module is mandatory –includes staff safety attitude questionnaire  All other modules are chosen based on facilities needs or aligned with corporate priorities  Once module is completed the requested information is sent to the Network Ex. Project Reporting form etc.

Details con’t 13  All participants completing at least one component or more will be recognized with a certificate.  Certificate will include either one diamond, two diamond etc to designate Diamond status.

Recognition Five modules completed = Five- Diamond Patient Safety Facility  Plaque to display in unit designating achievement of Five Diamond status  Acknowledgement in Network Newsletter  Listed on Website  Network encourages facilities to market themselves as a Five-Diamond Patient Safety Facility.

 The following schedule applies to Five- Diamond Status: Achievement between 1/1/2011and 1/1/2012 is valid until 1/1/2013  Providers may renew their diamond status for another year by completing two additional modules. 15 Five-Diamond Status Schedule

Benefits  Value added project for Network to promote provider staff development and education  Supports national CMS goal to promote patient safety  Favorable to the DOH  Stimulates more creative projects at the provider level

Network Goals 10% of Network 2 facilities will be registered and 50% of those facilities will have achieved at least One Diamond by the end of

Where are we Now?? 79 facilities (32% of NW2) have registered to the Program 30 facilities have achieved at least one diamond 1 5-Diamond Patient Safety Facility Facility status are updated regularly on our website Recognition consent form is requested by each participant of the program 18

What’s next? Continued communication and follow up with facilities who registered Modules are being updated. Continued marketing of the project via mailings using our brochure Reaching out to providers via s, phone calls and Webex demonstrations 19
