Fields to be filled 2. College Code: Write your college code as shown in the box For Example: If the college code is 1001 then fill the boxes as shown and the circles should be marked as follows. Get the college from the college office.

Medium and gender 3. Medium: Write your Medium as shown in the box For Example: If the medium is Kannada then fill the boxes as shown and the circles should be marked as follows. 4. Gender: Write your Gender as shown in the box For Example: If the Gender is Male then fill the box as shown

Fields to be filled 5. RELIGION Write your Religion as shown in the box For Example: If the Religion is Hindu then fill the box as shown 6. Caste Category: Write your Caste Category as shown in the box For Example: If the Caste Category is GM then fill the box as shown

Fields to be filled Physical Handicappe d: Write if Physically Handicapped For Example: If No then fill the box as shown

Register Number If the Register Number is 08A10101 then fill the box as shown If register no is of 8 or 9 characters and all characters are numeric, then fill the register no in this block. If your register no if of 9 characters then eliminate the first character and fill in the remaining 8

10. Course & Semester Code Write your Course and semester as shown in the box. Note: For course codes see Annexure – I. Subjects for the examination The subjects should be marked continuously, Before you fill subject code, recognize the type of subject A, B, C, … semester 01, 02, 03, … subjects Status: most important, it gives the information, appearing not appearing etc..

Signature I/We confirm that the information given in this form are true and correct as per the admission register maintained in this college All the formations are given in the “”how to fill the OMR form” booklet. The college code and the subject codes can be displayed on the college notice board Copies can be made and given to the students for their easy reference. Better if the students are made to fill the xerox and then the original.

Answer book Name block is removed Bar coding is introduced Answer book number is available below the barcode In the junior supervisor report write that number. The bar coded front sheet will be removed after first valuation FOR ENTERING THE MARKS TO THE COMPUTER. Tracing of answer book, for revaluation through the answer book number. Students should not scratch or make any ink mark on the barcode.
