Support Planning Using Person Centred Approaches

Looking for the best way to get the information from the person and people close to them without interrogating them. “It’s like having a conversation with your friend about what they’ve been up-to you do it because you have genuine interest”.

“Planning that was more free flowing natural and relaxed”. You get to see things in action, rather than the person telling you. “End up with a better quality of information”.

Respect Choice Ownership Support Opportunity These are the values and beliefs' we hold dear within the support planning process.

Be Honest –  with the people we support to plan  with ourselves  about the promises we make  about the outcomes

Support planning is about: “Discovering together personal outcomes for the person”.. What this means: An outcome is something that you want to change relating to your needs to make your life better.

In support planning you control the process not the answers…… Things you’ll need to find out before planning ………. What part(s) of the planning does the person feel confident to do themselves? Who does the person want to help with the plan? Where do they want the planning to happen?

What kind of planning session(s) does the person feel are best for them? Do they want to make any sort of decision-making agreement? Does the person need support with communication - Is a communication chart needed? Does this person have a learning disability, if so, do they have a person centred plan? More Things you’ll need to find out before planning ……….

The 7 Stages to Support Planning 1.What is important to you? 2.What do you want to change or achieve? 3.How will you be supported? 4.How will you use your individual budget? 5.How will your support be managed? 6.How will you stay in control of your life? 7.What are you going to do to make this plan happen?
