Appendix 7 South Eastern Trust Julie Kilpatrick and Rosie Kelly

Current Position Currently 55 children with complex health care needs in SEHSCT (17 score between 30 and 49 and 38 children score above 50) These children will have various complex health problems to include technological support, complex neurological conditions, palliative and end of life care.

Complex needs Majority of these children will start their journey in RBHSC Current Discharge Pathway—UHD step down or discharge directly home Holistic assessment of needs will be ongoing and support will be provided to meet these needs, by multi disciplinary team, both in acute and community (In- reach/outreach)

Impact of TYC Development of a skills set which facilitates children’s nurses to work between the acute and community setting Competency framework for high dependency care Skills set to support children moving into adolescence and onwards into adult services Joint service needs analysis based on actual and projected needs and this will inform the skills and knowledge base required.
