1 MUST be covered by a BLUE Medical Certificate 2 Certificates MUST be presented within FIVE (5) days 3 Guardians are ALWAYS informed if students leave school early due to illness 4 Students are ADVISED to present Medical Consent Forms for any Medical conditions that they may suffer from

1 Hospital/Health Centre Appointments MUST be covered by a Blue Medical Certificate 2 Students MUST present Proof of Attendance at Hospital /Health Centre Appointments stating time of entry / exit 3 Medical Appointments at PRIVATE DOCTORS / PHARMACIES MUST be carried out AFTER School hours.
